Lost PNG School Certificates, Diplomas, Certificate Replacement and Statement of Results

 If you have lost your  Papua New Guinea School Certificates (Grade 8 Certificates, Grade 10 School Certificates, Grade 12 School Certificates, Technical & Vocational School Certificates ,  Teachers College Diplomas), follow the instructions below and download the application forms on the links provided  at the end of the post. 

Take note:

  • Original Certificates are not reprinted second time round. Only Statement of results are produced. This is for Grade 8, 10 and 12 school certificates. 
  • For For Diplomas and Degree papers, read the explanation outlined below.
  • A fee for K25 must be paid to nearest finance department/Treasury Office (Grade 8, 10 & 12 Certificates)

How to get replacement certificates for lost Grade 8 School Certificates

 The Grade 8 School certificates are issued to students who have completed Grade 8 at the Primary School level throughout Papua New Guinea. The certificates are issued by the PNG National Education Department through the Measurement Services Division (MSD). In order for a student to be issued the Grade 8 school certificate, the student must meet the National Education Requirements by sitting the Grade 8 Examinations in the following subjects

  • English Language  Paper 1 (ELP 1)
  • English Language Paper 2 (ELP 2)
  • Mathematics
  • Combined Subjects 
After the Grade 8 examinations, the results are put on the school certificates and issued to the students through the school they last attended.  The Grades  put on the Grade 8 certificates are explained. 

  • VHA  - Very High Achievement
  • HA  - High Achievement 
  • SA - Satisfactory 
  • LA - Low Achievement 

Lost Grade 8 Certificates  Replacement  and renewal

If you loss your certificate, follow the steps below to get a Statement of Results. 
  1. Consult your Provincial Examination Supervisor (PES) in your province. 
  2. Fill out an application form if available. 
  3. Declare at a court house by filling out a Statutory Declaration Form that you lost your school certificate. Give this document together with your letter to your provincial PES. 
  4. PES will issue you a Statement of result.  (There is no reprint of original certificate once you lost your certificate). 
  5. Fees may apply (At the moment,  fees for Statement for Results for lost Grade 8 Certificate do not apply).

How to apply for  Statement Results for Lost Grade 10 School Certificate

The Grade 10 school certificates are issued to students who have completed Grade 10. The students must meet the National Department of Education requirements to be issued a Grade 10 Certificate. Grade 10 students must have sat the Grade 10 Examinations in  seven (7) of the many  subjects taken at grade 10 level. Below are subjects Grade 10 students study.

  • Personal Development 
  • Mathematics
  • Science
  • Social Science
  • Business Studies
  • Agriculture
  • Arts
  • Computing
  • Design and Technology
  • Practical Skills 
  • Home Economics. 

The Grade 10 School Certificates carry the following Grades

  • Distinction (D) 
  • Credit (C)
  • Upper Pass (UP)
  • Pass (P) 
  • Fail (F) 
Lost Grade 10 School Certificate.
If you lost your school certificate, follow the instructions below to apply for a Statement of Result. There is no reprint of original certificate once it is lost. Only Statement of Result is printed that contains all your marks. 

  1. Download the application Form on the link below (at the End of this post)
  2. Fill out the application Form. (You need to attach ID photos of yourself). 
  3. Declare at a courthouse that your lost your school certificate by filling out the Declaration Form.
  4. Pay a K25 fee at nearest  treasury office / Finance department and attach the receipt with your application  form.  Make sure to keep a copy of your application.
  5. Lodge your application form at Measurement Services Division located within the PNG Education Institute (PNGEI). It will take a week to process your application form. If you are outside of Port Moresby, you can send the original application form to . Grade 10 Principal examination Officer. How to apply for Statement of Results for Grade 12 Lost Certificate 

The Grade 12 School certificates are issued to students who have completed Grade 12 and have met the National Department of Education requirements. Grade 12 students must sit for  the Grade 12 National Examination . The must sit for examinations for  five (5) subjects in all the subjects offered at Grade 12 level.

Lost Grade 12 Certificate Replacement - Statement of Result

  1. Download the application Form on the link below (at the End of this post)
  2. Fill out the application Form. (You need to attach ID photos of yourself). 
  3. Declare at a courthouse that your lost your school certificate by filling out the Declaration Form.
  4. Pay a K25 fee at nearest  treasury office / Finance department and attach the receipt with your application  form.  Keep a copy of your application.
  5. Lodge your application form at Measurement Services Division located within the PNG Education Institute (PNGEI). It will take a week to process your application form. If you are outside of Port Moresby, you can send the original application form to . Grade 10 Principal examination Officer. Statement of Result Application Forms and how to apply

APPLICATION FOR STATEMENT OF SCHOOL RESULTS
INSTRUCTIONS FOR APPLICATION
If your grade 9 intermediate, grade 10 School Certificate or grade 12 Higher School Certificate is lost
or damaged, then you may use the attached application form to apply for a Statement of Results.
If your certificate is lost or stolen, you must report it to the police.
This is how you should complete this application.
1. Complete part 1 yourself. Attach your passport size photo to the application using some glue.
2. An official who has known you well must complete part 2. This official must be a senior  education officer or church minister or a senior Government officer. This officer must also sign
and stamp across the bottom half of your photograph.
3. Complete the statutory declaration and sign it in the presence of the Commissioner of Oaths.
The Commissioner must also sign and stamp the declaration. There should be no correction on the declaration.
4. Take the completed application and statutory declaration to a Government Pay Office and pay a
fee of K25.00 and obtain a receipt. You must tell the paying officer to write MSU trust account
Code 361-08 (639) on the receipt. This will be effective from May 5, 2003.
Those who live in NCD and central province should pay the fees at the government pay office in
Vulupindi Haus or the Accounts Section of the Department of Education on the 
Those who live in other provinces should pay the fees at the Provincial  government pay office (BMS).
5. Staple the receipt to the completed application form and the statutory declaration.
6. Send original receipt together with the completed application form and statutory declaration to
the following address by hand or mail.

If any of the information given on the application is not correct, the application  will be returned to the applicant and no statement will be processed.
Take note that the fee of K25 is not refundable under any circumstances,  therefore, you must ensure that the application is completed correctly and  truthfully.

If you lost your Diploma papers for those Teachers Colleges, Technical colleges and Nursing colleges, follow the following steps.

  1. Report the lost/stolen certificates to authorities, that includes, Police, Teaching Service Commission for Teachers/ Health Department for Nursing colleges. 
  2. Write to Department of Higher Education, Research, Science and Technology (DHERST) and make your submission for your Diploma reprint.  

Degree Papers
If you lost your degree papers (Bachelors, Masters, PhD ) issued by PNG tertiary institutions, please consult the issuing institution or the Department of Higher Education Research, Science and Technology. (DHERST)
